LPG and AIFG have worked together to combine carriers to provide access to a fully customizable benefits package on an A Le Carte basis! You will be able to utilize company funds and pre-tax dollars to choose benefits you and your family actually need! In this offering you have a Group Health Plan, ICHRA, Defined Benefits, Ancillary Benefits, Pension Whole Life Plan, as well as a 401(k) and Roth 401(k) to help you achieve your goals. Let's get started!

Consider: Any current plans or benefits you, or your spouse, may already have in place. Flexible Benefits allow us to fill any gaps in coverage that may exist.
Schedule: Click the "Schedule Benefits Call" button then "Discovery Call" to schedule a phone call and go over the plans with the benefits coordinator, as well as enroll in plans! Brian will be available for in person consultations with an in office service day every month, however, it's best to enroll as early as possible and we want to make sure you're taken care of promptly!
Cigna PPO Plans
Employees who are 35 or older, or turning 35 in the current plan year, can choose from three Cigna PPO plans. Each plan gives access to a broad network of doctors and hospitals and helps cover medical costs for routine care, prescriptions, and more.
Plan 1 – $5,000 HSA: This plan has the highest deductible, which means you pay more out-of-pocket before the plan starts covering costs. Because of the lower monthly premiums, it can be a good choice if you are generally healthy and don’t expect frequent medical visits. This plan is eligible to pair with a Health Savings Account (HSA), which allows employees to save pre-tax money for medical expenses if they choose to open an HSA independently.
Plan 2 – $3,500 HSA: This plan has a moderate deductible and monthly premium. It balances the cost of premiums with out-of-pocket expenses, making it a good middle-ground option. Like Plan 1, it is eligible for an HSA.
Plan 3 – $2,500 Co-Pay: This plan has a lower deductible and uses set co-pays for doctor visits and prescriptions, which means you generally know what you will pay at the time of care. It offers more predictable costs and can be a good option if you prefer budgeting your health care expenses.

ICHRA
Employees 34 and under at LPG Management have the option to receive health coverage through an Individual Coverage Health Reimbursement Arrangement (ICHRA). This gives employees the freedom to choose their own health plan, instead of fitting their needs into a one-size-fits-all group plan. Employees can pick a plan that best meets their family’s health needs and life stage.
Because the plan is individually owned, it’s important for employees to take responsibility for managing their coverage. Make sure to download your plan’s app, keep your insurance card up to date, and set up any necessary auto-pay for premiums. With a little attention, these plans can be very flexible and work well for a wide variety of needs.
LPG Provides $350/Individual & $700/Family monthly to use toward your plan, anything above this amount is a pre-tax deduction.
Ameriflex provides an ICHRA debit card for you to use on premiums and 213d Medical Expenses (if funds are left over after premiums are paid)

Defined Benefits
If employees choose to waive both the Cigna PPO and the ICHRA, they may be eligible for the Manhattan Life Affordable Choice Plan. This plan is offered at no cost to the employee, providing a safety net for those who do not select the other coverage options.
Because this is an individually underwritten plan, eligibility is subject to medical underwriting by Manhattan Life. Employees must actively enroll in this plan and meet the underwriting requirements to receive coverage.
This option is designed for employees who prefer a simpler alternative or who do not want the other plans. It’s important to review the plan documents and understand the coverage details before enrolling.

Ancillary Benefits
Ancillary benefits can help fill the gaps. These Products are designed to complement your health insurance by helping you with the expenses that your health care plan doesn’t cover, They include Dental/Vision, Accident, Hospital Indemnity, Cancer, Disability, Critical Illness, and more. Unlike many insurance products, benefits are paid directly to you, so you can use them however you like.
LPG is providing $80/month for you to spend on benefits within this offering, any premium beyond that will be administered on a payroll deduction basis.
With these plans you can cover yourself, spouse, children, and even Grandchildren!
This offering contains Dental/Vision, Disability, Life, Hospital Indemnity, Accident, Cancer, & Critical Illness.

Voluntary Whole Life
New for this year is a voluntary Whole Life policy offered through Lafayette Life. This is your opportunity to elect premiums from your payroll for Whole Life policies that build cash value over time from a dividend paying company. This benefit can begin on any first of the month after plan approval (subject to underwriting), it is post-tax, and can insure yourself, your spouse, children, grand children, AND parents'; so long as an insurable interest exists. Premiums are subject to underwriting. Please review the concept and product brochures in this section.

Retirement Plans
Your retirement plan through LPG consists of a few parts, consisting of a traditional 401(k) and a Roth 401(k) for your consideration, and the company is issuing a profit-sharing program as well. It is extremely important that you access this information on a desktop, as this is how Capital Group best encourages employees to enroll. Here you will find a video to watch that describes how to enroll, for your convenience there is a pdf version walking you through the entire enrollment process. IMPORTANT: The retirement program is a self-enrolled benefit, be sure to follow the below instructions accordingly. There is an automatic 3% enrollment if you do not enroll and select a different deduction amount, or waive the benefit with the election/waiver form.
